Once you have made your initial appointment, you may pre-register by downloading, printing, and completing the forms below. Please be sure to bring them with you to your first visit. Completing the forms prior to your visit will help speed up the new patient registration process.

At Union Square Medical Associates, we accept Medicare and most major insurance plans. Please contact your insurance company or you may call our office at (908) 352-9556 or (908) 282-6474 to find out if we accept your insurance plan.

If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, MasterCard, Discover, and American Express.

If one of our doctors suggests a specialist evaluation or diagnostic testing that requires a referral, we will make every effort to provide this for you as soon as possible. Some referrals require insurance preauthorization and cannot be processed immediately. Insurance guidelines prohibit us from backdating referrals. We require three business days for the processing of routine referrals.

In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Union Square Medical Associates, and they will contact us.

If you have an urgent medical need and need to talk to our doctors after our regular hours, please call our answering service at (844) 278-2271. A doctor is on-call 24 hours a day. Your call will be returned promptly.

Doctors at Union Square Medical Associates are affiliated with Trinitas Regional Medical Center, Elizabeth, New Jersey.

Prescriptions and refills for established patients of Union Square Medical Associates are routinely issued during our regular office hours (see above). You can make your request by calling our office at (908) 352-9556 or (908) 282-6474. No refills are issued during the evenings, weekends, or holidays, as the doctors do not have access to your medical records during those times. It is helpful to anticipate your refill needs to avoid an interruption in your medication therapy.
